Application & Reporting Forms

This page has many of the forms needed to manage project (competitive) grants from the Historic Preservation Fund. The information below outlines the form name, form number (if any), as well as additional information that may help you complete or submit the form. Forms are broken down into two general categories: forms to complete an application for a grant and forms for managing a grant. Please note that all application materials, including the forms referenced here, are submitted via grants.gov. Forms for managing your grant are never submitted through grants.gov, but may be submitted via email or via GrantSolutions. You should work with your grant manager to determine the best method for submitting forms once you have received a grant.

Assurances for Non-Construction Programs SF-424B You are applying for a grant.
Budget Information for Construction Programs SF-424C You are applying for a grant that includes development (construction) work. The total on this form should be included in the SF-424A.
Assurances for Construction Programs SF-424D You are applying for a grant.
Disclosure of Lobbying Activities SF-LLL You are applying for a grant.
Project Description You are applying for a grant and need to describe what grant-assisted work will include. funding opportunity announcement on grants.gov You will download a copy of these forms from the Notice of Funding Opportunity posted to grants.gov and upload completed versions into your workspace in grants.gov.
Budget Justification Form You are applying for a grant and need to justify the funding amount requested.
Photo You are applying for a grant and need to provide images of the historic resource(s) to be assisted by grant funding.
Certification of Staff and Review Board You are submitting an application for an annual formula grant on behalf of a State Historic Preservation Office. this link You will download a copy of this form and have an authorized individual complete it. You will then attach it to your application for an annual formula grant using the attachments form. Note that only State Historic Preservation Offices complete and submit this form as part of their annual formula grant applications.

Budget Information for Non-Construction Programs SF-424A You are reporting on a THPO formula grant grants.gov Detailed instructions provided in program-specific guidance.
Federal Financial Report SF-425 You are reporting on what you have spent under your grant award within GrantSolutions Instructions on how to complete the SF-425. Beginning in 2021, this should be completed directly in GrantSolutions where it is called the FFR.
Tangible Personal Property Report SF-428 You have purchased reportable property and you need a coversheet (optional) grants.gov Instructions on how to complete the SF-428 family. This is an optional coversheet for the other 428 series forms. Beginning in 2021, this can be uploaded directly into GrantSolutions as an attachment to the PPR. The SF-428 series forms are only required if you have purchased reportable property.
Tangible Personal Property Report - Annual Report SF-428A You have purchased reportable property and you are submitting an interim report grants.gov Instructions on how to complete the SF-428 family. Beginning in 2021, this can be uploaded directly into GrantSolutions as an attachment to the PPR. The SF-428 series forms are only required if you have purchased reportable property.
Tangible Personal Property Report - Final Report SF-428B You have purchased reportable property and you are submitting a final report grants.gov Instructions on how to complete the SF-428 family. Beginning in 2021, this can be uploaded directly into GrantSolutions as an attachment to the PPR. The SF-428 series forms are only required if you have purchased reportable property.
Tangible Personal Property Report - Disposition Request/Report SF-428C You have purchased reportable property and you are requesting to dispose of property before a grant ends grants.gov Instructions on how to complete the SF-428 family. Beginning in 2021, this can be uploaded directly into GrantSolutions as an attachment to the PPR. The SF-428 series forms are only required if you have purchased reportable property.
Interim Progress Worksheet You are submitting an interim report on a project grant PDF Beginning in 2021, this can be uploaded directly into GrantSolutions as an attachment to the PPR.
Final Project Worksheet You are submitting a final report on a project grant Word Beginning in 2021, this can be uploaded directly into GrantSolutions as an attachment to the PPR.
Final Project Worksheet (disaster grants) You are submitting a final report on a disaster grant Word Beginning in 2021, this can be uploaded directly into GrantSolutions as an attachment to the PPR.
NHL documentation You are proposing a project affecting a National Historic Landmark. This is not a form; it is a list of required information to be submitted to your grant manager for projects affecting National Historic Landmarks. SHPOs should submit this information through HPF Online for formula or disaster grants. For disaster grants, THPOs should submit this information through HPF Online.
NEPA Screening Worksheet You are proposing a development (construction) project or archeological investigation with ground disturbance. Instructions on how to complete the NEPA Screening Worksheet. Note: For disaster or formula grants, SHPOs should complete this worksheet within HPF Online. For disaster grants, THPOs should submit this information through HPF Online.
Annual Accomplishments Report You are a THPO reporting on an annual formula grant. More information on managing an annual THPO formula grants.
FFATA You are reporting on a subaward greater than or equal to $30,000 (formerly $25,000) Helpdesk provided through the Federal Service Desk. FFATA is the Federal Funding Accountability and Transparency Act; this act created FSRS.gov which is the system where reports are filed.
Easement / covenant / preservation agreement You are undertaking project work that involves development (construction) or ground-disturbing archeology. Model easement and more information.
